Insurance & Medicaid Coverage for Doula Care

HSA/FSA usage, Medicaid eligibility, and tips for reimbursement

We believe your care should be accessible — including when you’re navigating grief, termination, or pregnancy loss.

While coverage varies widely, here’s what you should know:

  • Some state Medicaid programs (including Michigan, California, Massachusetts, Illinois) cover doula support for miscarriage, stillbirth, abortion and bereavement services — you can ask your plan if you’re enrolled in one of them.
  • Private insurance plans may or may not include doula services. If your plan treats “emotional, non-clinical support” as outside of standard coverage, you may be eligible for reimbursement or using other benefits (HSA/FSA) instead.
  • Before beginning services:

    1. Ask your insurer: “Does my plan cover doula services in the context of pregnancy loss, abortion or bereavement?”
    2. Confirm whether your doula is credentialed or registered with your plan (or whether you’ll pay up front and request reimbursement).
    3. Request an itemized receipt + service codes from your doula to use for claims or HSA/FSA reimbursement.

When you work with us, we’ll provide you the documentation you need (service invoices, descriptions, codes) and help you explore whether your plan covers the session or if HSA/FSA will be a better fit.

Reach out with questions — we’ll help you chart the path forward.

How to Check if Insurance Covers Doula Support for Abortion or Bereavement

Even though coverage varies, many families can still use insurance, Medicaid, or HSA/FSA funds for these services.

Follow the steps below to find out what’s available to you.

Step-by-Step Coverage Guide

Confirm Your Insurance Type

  • I have private insurance (Aetna, United Healthcare, Blue Cross, Kaiser, Providence, etc.)
  • I have Medicaid / Oregon Health Plan (OHP) or another state plan
  • I’m self-paying and may use HSA/FSA funds

Ask These Key Questions

Call the number on the back of your insurance card or contact your case manager and ask:

  1. “Does my plan cover doula services related to pregnancy loss, abortion, or postpartum care?”
  2. “Are there any requirements (such as referral codes or a Letter of Medical Necessity)?”
  3. “Does my doula need to be credentialed or registered with your plan for coverage?”
  4. “What documentation do you need for reimbursement?”

Check Your State’s Medicaid Options

If you’re on Medicaid, see if your state offers perinatal or bereavement doula coverage.

  • California, Massachusetts, Minnesota, Oregon, Illinois, and several others have programs that include loss and abortion support.
  • Oregon’s OHP includes full-spectrum doula services — including miscarriage, stillbirth, and termination support.

If you’re in a participating state, ask your doula if they’re enrolled as a Medicaid provider.

Use Your HSA or FSA Funds (If Insurance Doesn’t Cover It)

If you have an HSA or FSA, doula support for pregnancy, loss, or postpartum recovery may qualify as a medical expense.

How to do it:

  • Request a Letter of Medical Necessity from your provider (OB, nurse practitioner, or midwife).
  • Pay your doula directly and submit your itemized receipt + LMN for reimbursement.
  • Keep all invoices for your tax records.

If You’re Paying Out-of-Pocket

We’ll provide:

  • A detailed invoice with service descriptions and CPT codes (if applicable)
  • Documentation for HSA/FSA or reimbursement submissions
  • Flexible payment plans if needed
