Using HSA, FSA or Medicaid for Doula Care

HSA/FSA usage, Medicaid eligibility, and tips for reimbursement

We believe every family deserves access to compassionate, holistic birth support — and we want to make sure cost isn’t a barrier. Here’s how you can use your HSA, FSA, or Medicaid benefits toward doula and perinatal services.

Health Savings Account (HSA) & Flexible Spending Account (FSA)

Yes, you can often use HSA or FSA funds to pay for doula and birth-related services!
Doula care may be considered a qualified medical expense when it’s directly related to pregnancy, birth, or postpartum recovery.

Here’s how to use your HSA/FSA for doula care:

  1. Contact your plan administrator to confirm eligibility. Ask if “doula services” or “labor support” are covered under pregnancy-related expenses.

  2. Request a Letter of Medical Necessity (LMN) from your provider (OB, midwife, or nurse practitioner). Most accounts require this form to approve reimbursement.

  3. Pay your doula directly using your HSA/FSA card, or submit your receipt and LMN for reimbursement.

  4. Keep all receipts — including consultation fees, deposits, and service invoices.

Tip: If your plan covers “prenatal or postpartum support,” you may be able to use funds for childbirth education, lactation, or postpartum doula care too.

Medicaid Doula Coverage

Medicaid coverage for doula services varies by state, but it’s expanding rapidly.
Currently, over half the U.S. states offer or are piloting doula reimbursement programs.

If you’re on Medicaid:

  • Check your state’s Medicaid website or contact your local managed care organization.

  • Ask whether doula services are covered under maternal health, perinatal, or community health worker benefits.

  • Some states require that your doula be a registered Medicaid provider — we’ll help guide you through that process if needed.

  • Even if your state doesn’t cover doula care yet, some community health programs or grants may help offset your cost.

Our Commitment

We’re committed to making birth care accessible, inclusive, and affordable.
If you plan to use your HSA, FSA, or Medicaid benefits, let us know during your consultation — we’ll provide an itemized receipt and any supporting documentation you need to submit for reimbursement.

Need help?
We can walk you through the process step-by-step so you can focus on your birth, not the paperwork.
